The use of digital mammary tomosynthesis with reduced compressive force during breast cancer screening resulted in higher sensitivity and lower specificity of lesion detection compared to 2-D digital mammography that can reduce the burden of radiation and screening load according to a study published in Lancet Oncology.

The Malmö Investigators Group for Breast Tomosynthesis Screening (MBTST, ClinicalTrials.gov Identifier: NCT01091545) conducted a prospective, based on a diagnostics population that compared the accuracy of digital mammography mammography in breast cancer detection.

Women aged 40 to 74 who were not pregnant were asked to take part in screening breast cancer in Malmö, Sweden. Eligible women enrolled in the study (n = 14,848) were screened with 2-sided digital mammary mammography followed by digital mammary tomosynthesis with 1 visualization and reduced compression in 1 breast cancer.

Detection sensitivity and specificity were the primary measurement result; Secondary results included screening performance for cancer detection, recall and interval cancer, as well as a positive predictive value for screen acquisition and a negative predictive value for both methods.

Researchers reported that digital breast tomosynthesis had a higher sensitivity (81% vs 60.4%) but marginally lower specificity (97.2% versus 98.1%) compared to digital mammography. A total of 8.71 cancers per 1000 women who were tested against 6.5 cancers per 1,000 women surveyed indicated that cancer detection was significantly higher in 1-fold digital breast tomosynthesis compared to 2-digit digital mammography.
