I met Professor Shin, Dong-hyuk of the Plastic Surgery Clinic at the Konkuk University Hospital, to discuss the problems and preventive measures of the Korean diabetic foot care environment.
– The reason why diabetic foot is dangerous.
“Diabetic patients often don't know enough to break sensory nerves. It is not easy to feel uncomfortable even when the skin is swollen and feels hot due to infection. – Is there an effective method of controlling infection that prevents cleavage?
"It is important to control wound infection in time to avoid leg cuts." It is useful to use a dressing that can remove abnormal tissue and improve infection. However, there are many obstacles to using effective formulations for dressings in Korea. The patient does not receive proper doses of treatment. Typical is a silver bandage. Silver-containing dressings are highly sterilizing agents. Silver directly penetrates bacteria and kills bacteria. It is effective in controlling infection. However, in Korea, only a dressing product is used that has a low sterilizing power since it is not approved for diabetic patients. Currently, silver-containing dressings can be used for burn patients, but it is illegal to use them for diabetic patients. This part is absurd. Since the diabetic foot is infected and can be separated by cutting, effective dressings should be used. We have to change the criteria so that they can be used not only for burns but also for infected wounds.
– Overseas dressings containing silver are widely used.
“There are thousands of papers on the effects of a silver bandage on infectivity. In 80% to 90% of the products, it is said that silver-containing dressings are excellent for reducing bacteria. The astounding result is that full recovery of the infection takes less time. There are many other dressings such as honey dressings and alcohol-containing dressings that are not introduced in Korea. Domestic use permits are too complex and inflexible.
– If there is a proposal to improve the home environment for diabetes.
“The goal of care for a diabetic foot is to prevent serious infections or to control infections so quickly that they don't go away. To achieve this, doctors should be able to choose a good treatment first. It is also important how often doctors walk and touch their wounds. We hope to create an environment for multidisciplinary treatment. A diabetic foot is a disease that several medical departments must see together. "The biggest prejudice about plastic surgery is that it's just cosmetic surgery." Plastic surgery covers a wide range of areas.
